We randomized adults (n = 34) with glycated hemoglobin (HbA1c) > 6.0% and elevated body weight (BMI > 25) to a very low-carbohydrate ketogenic (LCK) diet (n = 16) or a moderate-carbohydrate, calorie-restricted, low-fat (MCCR) diet (n = 18). All participants were encouraged to be physically active, get sufficient sleep, and practice behavioral adherence strategies based on positive affect and mindful eating. At 12 months, participants in the LCK group had greater reductions in HbA1c levels (estimated marginal mean (EMM) at baseline = 6.6%, at 12 mos = 6.1%) than participants in MCCR group (EMM at baseline = 6.9%, at 12 mos = 6.7%), p = .007. Participants in the LCK group lost more weight (EMM at baseline = 99.9 kg, at 12 mos = 92.0 kg) than participants in the MCCR group (EMM at baseline = 97.5 kg, at 12 mos = 95.8 kg), p < .001. The LCK participants experienced larger reductions in diabetes-related medication use; of participants who took sulfonylureas or dipeptidyl peptidase-4 inhibitors at baseline, 6/10 in the LCK group discontinued these medications compared with 0/6 in the MCCR group (p = .005). In a 12-month trial, adults with elevated HbA1c and body weight assigned to an LCK diet had greater reductions in HbA1c, lost more weight, and reduced more medications than those instructed to follow an MCCR diet.
  2. It makes sense to me. Better to eat "real food" (i.e. unprocessed) and using nutritious ingredients rather than the VLCD shakes, bars, soups and desserts which have come out of a packet, and are a food-like product. Personally I'd much rather have a salad with some protein rather than eat those products. Better to eat "real food" (i.e. unprocessed) and using nutritious ingredients rather than the VLCD shakes, bars, soups and desserts which have come out of a packet, and are a food-like product. Personally I'd much rather have a salad with some protein rather than eat those products. Believe me, there have been some hideous outcomes of folks who have had the gastric sleeve done, and then have experienced horrendous outcomes, through no fault of their own. And despite following every guideline ... It is SERIOUS surgery. And needs to be treated, and respected as so. Please - just fork out the dosh, and book in and follow the guidelines of an experienced bariatric dietitian etc. It takes 10/52 for the stomach to heal, and even after that, late 'leaks' can happen. And other complications ... It is up to YOU , BEFORE you have "Weight Loss Surgery" ("WLS") to research, research, research. Read, read, read. There are numerous resources out there. Prepare, prepare, prepare. Remember "The Six P's"? I am not wishing to be crude here, but a good 'rule of thumb'/guideline is "Prior Preparation Prevents Piss Poor Performance". This DOES not replace the guidelines and adbice of your . That is their area of speciality - not your faceless peers. And that YOU have to be accountable. You. Nobody else. Believe me, WLS is just a tool. If you don't use it wisely, then it will not work for you (so many posts about "I am a lapband/gastric sleeve/gastric by-pass failure".) And my personal experience. I HAVE to be diligent. Hour by hour. Day by day. Week by week etc. And, at the age of (turning 54) 53, I HAVE to follow a "Low-carb, healthy fat" diet (LCHF). Otherwise I just "blow-out". Many, many years ago - i.e around year 2000, I did that just that, No excuses for having your potentially-life threatening surgery done, and THEN ask "What should I be eating?" *sigh*. I just 'don't get it'. Why you haven't you already connected with a bariatric-savvy dietitian BEFORE having this operation done? .... And then you have read all the other resources out there? Totally frustrating. Us long-term sleevers answer queries such as this SO SO many times, then we get 'burnt out'. Don't have the emotional/physical/well-being to answer the same questions over...and over ...and over ... and over again etc. So, at the end of the day, YOU have to be accountable. I don't dish out platitudes - because, even though this may give that person instant gratification - What? - at the end of the day? - is best for them?, and what they are seeking advise about? Instant gratification? or genuine advice?. And it all boils down to head space. Those who (superficially, and nominally) seek - "advice" from their peers on forums such as these, rather than from experienced and qualified health professionals); unless they are willing to change head space/attitudes/beliefs - (and "In My Humble Opinion" " IMHO") - is that for those folk. please seek professional help, who specialise in eating behaviour disorders. Rather than need constant 'stroking' of their egos. Because in 1/5/10/15/20/ 20 + years, what is better for them? (in 'normal mental health'... , and if they have other mental health/personality problems, such as "borderline personality", it isn't us folk to try to work with this), it is their "health-care professionals". Please be careful, for all of us "helpers" out there, there are are a lot of folk out there with "borderline personalities". Just speaking from experience ... All the best!

It has suddenly dawned on me that after 18+ months on this journey that I just simply do not like most of the foods I used to eat and I've discovered a whole new world of wholesome, healthy and DELICIOUS foods that I never knew existed. Some of my new found faves (OK, obsessions) include: Home made hommous... eaten by the tea spoon Sweet potato... any way I can have this is fine with me... baked, grilled, pan fried. I have it with EVERYTHING, even my salads haha. Protein shakes/home made protein bars/balls/pancakes Coffee. I always thought I liked coffee but in reality I was masking the taste by having Mocha or a cap with lots of sugar. I now have a skim flat white, no sugar and OH MY... the coffee taste What are some of the awesome healthy foods you've discovered since your health/WLS journey? I am looking to expand my horizons even further! Joujou

Well after a few set backs, I have started training with a PT about 4 weeks ago...on usual diets I would loose on the scales but this time I am in shock and haven't lost much by weight but lost 20.5 cm off my body...he said this is the best way to keep my metabolism working when there is muscle building...the fat will slowing disappear to ...I have always worked the other way looking at scales every week now it hard to get my head around , feeling the muscle building though...

    11 days post op - disappointed

    Had a few disappointments this week, which have really taken the shine off having the band. First up i had my 1st pre-op appointment on tuesday. The actual appointment when really well, my nurse was great and really supportive and said i was doing fantastically. It was one week post-op and she said that if i was feeling hungry on the fluids and was ok with starting pureed i could go ahead. We discussed what i should and shouldn't be having and i left on such a high knowing that i had recovered very well and that i was progressing better than expected. It went down hill from there. On my way home i stopped off at the shops and bought avocado, dip and some light cruskits (i figured chewing them well with dip they would break down fine). I thought i might like to try some fish as well, so i called up the clinic to see whether that would be ok. The nurse wasn't there, but the practice manager was, and when i told her my query she immediately shot me down, saying i should be still on fluids and definitely not even thinking about anything more solid for another week. When i told her how hungry i was and how i was having quite alot of fluids, she suggested i have an extra optifast and that should fill me up. When i said that i wasn't having any optifast at all - i was following the advice in the book they recommended which had a bunch of pureed soups in the liquid phase and my multivitamins - she was very agitated with me and told me i should follow the dieticians advice to avoid damaging my band. I was unsure whose advice to take and she said she would call the surgeon and tell me what he said. By this time i was home with what i had bought and thought i would try it anyway. I had two cruskits with avocado and lemon, chewed very well and paused between each bite (that slow eating app is great!) Not a problem. No pain, no discomfort and i felt pleasantly full but not stuffed. Then the manager rang back and said that the surgeon confirmed what she had told me, and i was to go back on fluids for another week. At this point i was pretty upset. I had been on such a high before, was obviously capable of eating mushy food, but now i had lost my confidence in anything the nurse had told me, and felt like i had been a failure because i might have damaged my band. I came on here and everyone has differing stories, one surgeons team says 1 week liquid, 1 week pureed then solids, but mine says two weeks of each. Plus i re-read my dieticians notes about the fluid diet and i discovered that what he constituted as pureed was alot of what i had been drinking in the liquid phase - drinking yogurt, very runny soups ect. Despite the fact that i was using a book my surgeons office had told me to get! So by midday thursday i was a mess. I felt like i had done everything wrong and i might have "broken" my band and stuffed everything! So i wrote my dietician a long email explaining what had happened and asking his advice - and he hasn't gotten back to me. Not only that but my last email to him, which was almost a week ago now, he still hasn't replied to either. All up i'm feeling really let down by my team, upset because i feel like i should have gone with someone else, and angry because what i was so excited about, is now making me anxious and worried everyday. To top it all off this afternoon a was looking at one of my laparoscopy scars, which are healing nicely, and found that one of them has a stitch poking out - the one right under my bra strap. I was told i didn't have any stitches at all, so WTF is it doing there? And its not a dissolvable stitch because 11 days out with everything else healed over, it should have been gone by now. I'm not very impressed with the whole experience thus far, dealing with my team after the surgery. My first appointment with the surgeon is in two weeks, but i think ill be getting on the phone monday and making an appointment sooner. Hopefully i can sit down with him and discuss these issues, and he can sort things out for me. Im 7.5 kgs down, so at least thats still going well. For now i'm having avocado and thin soups, and i've put a bandaid over my stitch to stop it rubbing on my bra. Fingers crossed things get better from now on. Will let you know how it goes, Jess

    Afternoon All Just wondering - given the number of diets we all must have done between us in the the past - has anyone tried to the Eat to Live program? I came across it the other day while looking for suitable vegetarian programs for women with PCOS. It makes a lot of sense to me, and being a past vegan, it actually makes me see how even though I thought my diet was quite healthy, I was still eating lots of processed foods. For the last few days I have been following the diet, with the exception of a couple of soy hot chocolates. For example, yesterday I had a banana/blueberry/spinach/water/x2 tsp of flaxseed smoothie for breakfast, followed by a lentil and pumpkin burger with salad greens for lunch, and a tofu & asian veg stir fry for tea. I had an afternoon snack of homemade hommus with a toasted wholewheat tortilla. This morning I had another smoothie, and morning tea of homemade hommus and chopped cherry tomatos and carrot sticks. I'm amazingly not hungry, for the first time in AGES! I have flatlined on the scales now for almost two months. In the last couple of weeks I have increased my exercise and still no movement but now magically in three days I am half a kilo lighter! Any thoughts from people who have tried the program? Peace, VM

    They have Dry July and Feb Fast for drinking, so I hereby create No Junk June for eating The goal for the month is to eat clean The Basic Principles of Eating Clean Eating clean involves not only choosing the right foods to eat but also avoiding all of the junk foods and processed foods that are so readily available. The keys to good health and proper nutrition are in the following principles: Eat whole foods: Whole foods are foods that haven’t been tampered with, in the lab or the manufacturing plant. The foods you eat on this plan are straight from the farm: whole fruits and vegetables, whole grains, grass-fed and free-range meats, low fat dairy products, unsalted nuts, and seeds. Avoid processed foods: Processed foods are any food that has a label. A label means that more than one ingredient was used to make that food. You don’t have to eliminate all processed foods (like whole grain pasta or natural cheeses), but if you can’t pronounce an ingredient on a label, don’t put that food in your shopping basket. Eliminate refined sugar. Refined sugar provides nothing but calories. Other sweeteners can be used, but with all the good foods you add to your diet, refined sugar really has very little place in the eating clean plan. Eat five or six small meals a day. By eating smaller meals throughout the day you can help rev up your metabolism. You never get so hungry on this plan that you’ll feel deprived or feel the need to cheat. Cook your own meals. Instead of buying meals in a box, cook meals from scratch. That’s not as hard as it sounds! Clean, whole foods need little preparation beyond chopping and sautéing to make satisfying, delicious meals your family will love. Combine protein with carbs. When you do snack or eat a meal, make sure that meal is balanced. For the most satisfaction from your diet, and so you’ll be less tempted to eat junk food, combine protein with carbs or carbs and fat. This simple act will fuel your body and quash hunger pangs The main issue I will have is giving up sugar in my tea and I loooove chocolate (I work for a chocolate company, eek ) Plus I use a lot of meal bases to make dinner so i will have to give that up as well. Lucky we dont eat take away or drink soft drink so I dont have to worry there! I have no doubt I will have a slip or 3, but it's something to aim for. For you all this may not be much of a challenge but I'm not banded yet so I want to get some healthy living in before I go overseas for a month.
  16. Hey Everyone, Thought I should put up another blog post since it's now day 15 and lot's has been happening. Have been on Optifast now for 15 days and I've now lost 12.2 KG's, 7.2KG first week and 5.0KG this week - so i'm pretty stoked to be honest. Have been putting in a lot of effort in regards to exercise, walking between 30 and 40 minutes every night or swimming for an hour and I go for a really long walk on Saturday night - 1.5hrs worth of walk. Diet is getting easier now, although I still want to gnaw my arm off sometimes. I just go exercise some more, or have a big drink which usually helps to get rid of said hunger - well at least enough to make it go into the background. Had my Sleep Test last Thursday night, so guess I will find out the results soon enough - here's hoping the sleep aponea has buggered off and I don't want to go back onto the CPAP machine. Superannuation check arrived in the mail last Friday, and has now been banked. So i'm all cashed up and ready for my operation. Expecting to hear back from the lovely gal's at CWLS sometime this week for a surgery date apparently. Thanks for all the support from everyone - such a lovely community here. Cheers, Craig
