    Measurement of obesity accuracy

    Calculating a patient’s waist-to-height ratio is the most accurate and efficient way of identifying whether or not they are at risk of obesity, a new study shows. The research, published in PLOS One, examined the whole-body fat percentage and visceral adipose tissue mass of a group of 81 men and women. The British authors discovered that 36.5% more adults would be classified as obese using whole-body fat data (one in two participants) rather than BMI (around one in seven participants, or 13.5%). To conduct their study, they gathered accurate whole-body and abdominal fat data using a total body dual energy X-ray absorptiometry (DXA) scanner — a highly accurate way of measuring body composition and fat content. They then calculated five predictors of whole-body fat and visceral adipose tissue that could be easily replicated in a GP’s office, and compared the results with those of the DXA scan to determine which simple predictor of obesity was the most accurate. The five predictors tested were: BMI, waist circumference (WC), waist-to-hip ratio (WHR), waist-to-height ratio (WHtR) and waist-to-height ratio0.5 (WHtR0.5). Lead researcher Dr Michelle Swainson, senior lecturer in exercise physiology at Leeds Beckett University, says although there are benefits to the conventional BMI method, there is concern that it is a misleading measurement. “This is most definitely the case when people have a 'normal’ BMI but high abdominal fat that is often dismissed,” Dr Swainson says. The results from the study show the best predictor of whole-body fat percentage and visceral adipose tissue in both men and women is WHtR. This simple method of waist circumference divided by height measurement is not a new obesity classification but, despite evidence supporting its use, it is still not routinely measured in clinical settings, the authors note. Cut-points for predicting whole body obesity were 0.53 in men and 0.54 in women. The cut-point for predicting abdominal obesity was 0.59 in both sexes

  6. It has been such a long time since I last posted here. I had first started my lapband journey on January 20, 2010 at about 148 kg. Now, five years later, weighing around 65 kg, I wrote in an English paper published in Turkey about what it was like living life as an obese person.     Here we go....

    So... Here we are in the month that I am going to have a lap band. I never thought that I would be here and right now, it feels strange and unreal. Years of struggling with weight, familial obesity, eating issues and bad habits have lead me here, there is no excuse any more and now I feel ready (per se). This week is all about the preoperative appointments, the echocardiogram stress test, the dietitian, the respiratory physician and of course the final pre op discuss with the surgeon, the man with the knife. I am doing this in secret and only one other person in my life knows about this. I feel alone, I feel like I am running without a destination at the moment, I feel like I have to choose the words that I say carefully, the excuses for postoperative pain already building in my head. Anyone who knows me would be overjoyed to hear that I am having this done, although my first choice was a gastrectomy, I believe that I have actually settled on the right choice for the moment; there are other options down the track if this doesn't work and I can adjust this if I need to, plus the bariatric physician that will do the fills is rather hot, so I think that I will enjoy that part. I fluctuate daily from excitement to fear to nonchalant denial, the endless papers to fill out, phone calls and appointments are tedious, but necessary I know. This sounds like I am whinging, that I do not appreciate what is about to happen to me with relatively little discomfort and personal injury/financial pain (first world problems, right?), but I am far from that in truth. I AM SCARED! I am afraid of the unknown, of being alone with this secret, of dumping and dilatation, of the loss of red meat and hair, the complications that may eventuate and most of all, FAILING MYSELF. This outlet helps considerably!
  11. Sorry for the cliched title. I loathe the word journey, hijacked as its become, but to be honest, I can't actually think of what else this is. A journey, an exploration, a study in self - it's many things - it's also a long overdue admission that I - Ms Very High Self-Esteem - am now so overweight that it's actually affecting my health. I can't be in denial any more, something needs to be done. I had lunch two weeks ago with my son's best mate's mum. I hadn't seen her for a while and she looked fantastic. I knew that in the past she had used Duromine to lose about 10-12 kg and thought she must have been on that but she said "didn't I tell you? I got a lap-band done in November. Best thing I ever did!". She has lost 11 kg (even over Christmas! I was very impressed) and reckoned it was so easy and why she didn't do it years ago she'll never know. I was fascinated. I listened hard to everything she told me and went home thinking "I think I need this". I am lucky to have a very supportive partner who loves me regardless of what I look like but all the same, I know he is very pleased that I've decided to do this. I made an appointment to see Prof. Paul O'Brien on 18 February but was excited to get a call saying there was an earlier appointment, so I went yesterday, 11 February. I was horrified to see what my weight was - 90 kg - BMI 35.6. I even find it hard to type that terrible number, but I knew it was time to face it. I am only 5'1" - 5'2" if I'm really lucky and 90 kg is appalling for my height. I had a hard enough time last year facing turning 50, then acknowledging I was actually going through menopause, now this. All these unfaceable things must now be squarely faced. Prof. O'Brien told me that my ideal weight is 63 kg (is it a sign? I was born in 1963...*cue spooky music*) and that I have 27 kg of excess weight but should aim to lose 18 kg to get to 72 kg. I responded "I look awesome at 70 kg". Like I said, I have a very healthy self-esteem So I got given all the paperwork and spent most of last night completing it - I absolutely couldn't complete the 40 page questionnaire in one hit so I did half last night and the rest this morning. I've had my GP sign the forms for the super fund, and tomorrow I'll post it all off. My surgery is scheduled for 24 March 2014 unless the super fund comes through quickly and there is an earlier spot, which I will take. I'm nervous and excited. I'm worried I will fail. I'm curious if I will succeed and if that success will be fast or slow. I fret that I'm a boredom eater and often eat when I'm not hungry. I wonder if I will still have the urge to buy a packet of biscuits or a block of chocolate and hide it in my office drawer to nibble on during the day. I am worried how I will cook for my partner and son when I don't feel hungry. In short, I have NFI what is ahead of me! If I'm brutally honest (and why shouldn't I be here? No one knows who I am, there are no expectations, I can truly be myself), I've always been a bit of a "happens to other people" type. But this time it's happening to me and I have to face it. I am fat. Horribly so for my height. I am sick of shopping in the fat chick's section. I am sick of not wearing sleeveless things. I'm sick of trying on bigger and bigger sizes to find something that fits. I've always had big boobs, but even I'm shocked at the sheer scale of the bras I'm wearing now. You could parachute safely to earth from a plane with one of them! I'm sick of seeing people I haven't seen for ages say "wow, you look really well" - when we all know that "well" is a weasel word for fat. Sometimes I even supply them with the word they can't bring themselves to say. I am nothing if not blunt. The Prof's letter to the superannuation fund, by necessity, has to be blunt, but man, it was really tough seeing the words "quite massive obesity" so baldly spelled out on the page. Well you know what? I never want those words used in relation to me again. So lap-band surgery it is. I really hope I can get an earlier appointment to have it done, just to get it over with so I can start this new...thing I don't want to call a journey. I know! A project. That's what it is - a project. LillyPilly's Fat Project. And it starts now. This would be great! I want to use this blog to articulate my thoughts as well as to share what's actually happening to me throughout The Project. I don't mind if no one reads this, it's mainly for me anyway, and I thank the Admins of the forum for providing a safe space to do this. Let us begin.

    Information is key...

    “Take a seat, shouldn’t be too much longer.†I sat down in the clinic, waiting for the start of the free information session. Yes I was a little nervous as I would be finding out a lot about what I am about to go through, but also I will be sharing something that has been a very private subject with all the other people in the room. Others stared to arrive and I began to question if I should even be here. It just seemed that everyone else was bigger than I, so I began to think is this something I should be doing because I don’t seem to fit that profile? The surgeon was running late, caught in theatre so the dietician started the session in hope that the surgeon would make it later on. She explained the basics of the procedure and what to expect. Explained about how this procedure will change our lives and some of the bad things you can do. It ended up being a really good session; people were sharing what they knew or heard, asking questions and clarifying wives tales. It was very refreshing to be amongst a group of people that were looking to start on the same journey as yourself. The surgeon had just arrived at the end of the session and gave a brief presentation on the risks and expectations of what we might be in for if we chose to continue. Again everyone was engaged and the information from our questions was valuable indeed. I would highly recommend attending these kind of free information sessions as there are a lot of questions you can get answers to before you even fork out the initial consultation fee. Walking out of the session I was even more confident that this was the right thing for me, now just to convince the surgeon…

    Step One - the GP

    There are a lot of things you hear about weight loss. Some good, some consider miracles and some that are just so bad you just want to ignore them. I think its just for the best to stay away from things with the word celebrity linked with them! I was at home recently, off work with the flu and I said to myself that why not! Why not take this opportunity to ask my doctor about banding, I was already going for a medical certificate anyway, may as well make the most of it. So off I went. While it wasn't one of the worst conversations I have ever had to have, it certainly wasn't easy. I'm not one that knows what a real therapy session is like, I normally can handle making sense of my life just fine on my own; but to actually say to someone else that you have a problem and you need help was the biggest thing I have ever done in my life. This coming from someone that is use to having to deal with his own problems and having to open up and to share what I and I'm sure so many you on this journey would consider, their darkest hour. It is hard to admit what you really eat. My GP is very understanding, very supportive of my goal. After the blood tests came back ok, just a slightly fatty liver and nothing else to worry about. We discussed other options and after laying out my life story he sent me away with that next nail blighting task, a referral to a specialist! It sat on the kitchen table for a few weeks, often I would just stare at it, thinking can I do this? Should I do this? It didn't help that some liked the idea and some thought it was a very drastic thing to undergo. But ultimately it is only you that can decide what is right for you, only you know the pain and the suffering that has brought you to this life changing decision. "Hello this is Sue." a soft but welcoming voice came over the phone. "Hi Sue, this is David. I have a referral and I would like to make an appointment." By this time again I was starting to think 'should I just hang up, no be strong, you have to do this'. "Ok great, the earliest appointment I have is in about a month and half, how about Wednesday lunchtime?" I was hoping to get in sooner, stop all that thinking and just get on with the action, now theres just more time to sit and ponder. Do I have a choice? Not really, coming so close to Christmas I just have to take what's available. Damn you referral, you sat there mocking me, making me question but eventually helped me to build up courage. "Oh really, he's that booked out, ok thanks, yes that should be fine." "Ok great I have booked you in with the specialist and the dietitian afterwards. We also have a group information session next week if your interested just to find out more about the procedures, would you be interested?" I needed to know more than what Wikipedia says and some of the horror stories I've Googled! "Yes that sounds interesting, please sign me up!" There it's done, you can relax and stop thinking about it."Ok thanks David, see you soon!"

    In the beginning...

    It was the best of times; it was the worst of times. Your friends still loved and supported you however as time went by it became harder and harder to find any clothes that fit, let alone any that looked any good. It was the simple things, the little things that broke the straw of my back. Not being able to tie a shoe lace easily, having to be careful to not put your back out getting out of the car, working up a sweat just walking up to the second floor. It is time. Most of my life I’ve been a big guy, starting in Primary School and the lunches from the corner shop. Starting out as one sausage roll and small Big M to two sausage rolls and a large Big M in the later years. I was a fussy kid, my friends would argue that I still am. Although I think the closer ones would agree that I have spread my wings and started to vary my palate in recent years. Before my school years I would live on frankfurts and mashed potato, hot chips and soft drink. My mother was also a big woman back then, while she did her best to keep me fed, I was too strong for her and she had no choice but to give in. Had I wished she stood firm, yes however I don’t blame her for it, stubborn kids are not easy to deal with and she just wanted to make me happy at the time. By 14 I remember that I was 80Kg, not sure of my height at the time so I can’t give an indication of the BMI I was back then, but I was big; big enough to be singled out at school, high school especially. While I would like to say that I have fond memories of my childhood dreams, there were also the not so pleasant experiences. It was expected, every fat kid gets picked on at school and I usually took it on the cuff of my shirt. It did bother me, but back then there wasn’t much I wanted to do about it and by the time I was a senior I was in a great group of friends that kind of shielded me from those that their only pass time was to barrage the emotional living daylights out of the venerable. By the end of high school I was about 110-115kg, 175cm, giving me a BMI of approximately 37, the Very High Risk category… By the time I finished University, living away from home I worked my way up to 122Kg and a BMI of 39.8, just a stone’s throw away from that life changing term… Morbid Obesity. Well at that point I made some sacrifices in my life and with the support of a very good friend of mine that played professionally in the NRL, supported and motivated me to make some changes. Well what a time in my life that was! Starting 4thMay 2009 I began a regimented routine of 6 small, relatively healthy meals a day along with around 2 physical boot camp style exercise sessions with my NRL buddy. On the 8th October 2009, just five months later I had lost 10.2Kg. I was a new person! I experienced designer clothes for the first time. I remember the day I was in Sydney and visited my first Industry store. I purchased a pair of size 40 (the largest they make) jeans. That was the best day of my life. I wore those jeans like a badge of honour. Sadly that was to come to a crashing halt and like all good things, they come to an end. I lost my focus and my motivation, but worst of all I lost my support. My buddy that was helping me got contract to play elsewhere and had to move away. I tried to keep up my motivation and determination however I feel that with a combination of not having any more one-on-one support, that someone to keep you motivated and focused, I lost mine. I tried to keep off the bad foods, keep riding to work but it all just fell apart and here I am, 12Kg heavier than when I started, that’s right folks, 134Kg with a BMI of 44, Morbidly Obese. While this is a sad time in my life, like the phoenix is reborn out of the ashes of its demise, I too will be reborn and starting now I have begun the mission of taking back my body, my life, my soul. Please join me while I take this journey of rediscovery and new heath through the miracle that is gastric banding and the transformation that is about to take place. Fighting the food fight, one band at a time.
